  • A completed Health History Form (submit online before your appointment)
  • Your dental insurance card

At Premier Periodontics, your appointment time is reserved just for you, so we can give you the focused, personalized care you deserve.

Here’s what to know:

  • Please provide at least 48 business hours notice if you need to reschedule or cancel your visit. Otherwise, a fee may apply
  • We set aside specific time for each patient, and advanced notice allows us to support others waiting for care

Running late? If you expect to arrive more than 5 minutes past your appointment time, please call us. We’ll do our best to adjust the schedule and minimize disruption to your care and others’.

We also offer interest-free financing through CareCredit:

  • 6-months deferred interest on treatments over $200

  • 12-months deferred interest on treatments over $1,000

Insurance and Benefits

Premier Periodontics accepts most major dental benefit plans and is a preferred provider for:

  • Delta Dental
  • Premera
  • Cigna
  • Sun Life (formerly Assurant)

To help us serve you efficiently, please submit your insurance information before your first visit, and remember to bring your benefit card to your appointment.

We’ll provide an estimate of your treatment costs based on the information your insurance provider shares with us. However, insurance plans vary, and the final coverage amount may differ from what’s initially quoted.

If your plan isn’t listed, don’t worry, we can still file your claim. You’ll be responsible for any portion your insurance doesn’t cover. Please note we do not bill medical insurance, only dental.
