Our goal is to provide exceptional periodontal treatment in a warm and caring environment. We strive to make treatment affordable by keeping our fees reasonable without compromising excellent care.

 

 

Dental Benefits (Insurance)

 

 

If you have dental benefits, we ask that you provide this information BEFORE your apppointment. This allows us to provide you with a treatment estimate the day of your exam. Also, please bring your dental benefit card to your first appointment.

 

 

We will be estimating your portion for any treatment rendered. Please remember this is only an estimate. Your dental benefit provider may choose to contribute more or less towards your treatment.

 

The estimated patient portion will be due at the time of service. You are responsible for any remaining balance not covered by your dental benefit plan.

 

We are preferred providers for the following insurance companies: (Some sub-plans do NOT participate please be sure to check with your benefit provider)

 

  • Delta Dental
  • Premera
  • Cigna
  • Sun Life Insurance (formerly Assurant)

 

If your insurance company is NOT listed above we can still process your dental insurance claim. You will be responsible for the amount your insurance company does not cover. (We do not bill medical insurances, only dental).

 

 

Please understand that insurance is a contract between you and your insurance company. We do our best to provide you with timely information regarding the cost of treatment, but you are ultimately responsible for all services provided by us.

 

The contract that you or your employer negotiated with your insurance carrier defines your dental benefits. We want you to get the most from your dental benefits, and this is best accomplished if you understand them. Please read your benefit or plan booklet or go online to your insurance company’s website. Various plans cover periodontal and implants procedures at different levels. As a result your payment portion may vary from our estimate.

We have negotiated an UCR (Usual, Customary, and Reasonable) rate with the above companies and will accept that rate. UCR rate is the term used by insurance companies to describe the amount they will pay for a particular periodontal/ implant procedure. You are responsible for a portion of the payment as defined in your particular contract. We do not know in advance the details of your contract. We, therefore, estimate your portion based on information provided by your insurance company and past experience with your insurance company.

 

If you do not have insurance, payment for treatment is due at the time of service.

 

 

We accept Visa, MasterCard, Discover and American Express as well as checks. If you have questions or concerns regarding the cost of treatment please ask our front office staff. We are happy to discuss with you the cost of treatment, and your various options for payment.

 

 

Payment & Financing Options

 

 

All dental fees are payable at the time of service. We accept Visa, MasterCard, Discover and American Express as well as checks. We also offer 12-month interest free financing for all dental procedures over $200 through CareCredit.
