This patient presented to Kirkland Premier Periodontics with a horizontal fracture of #9. The dentist temporarily cemented the crown back on, but the crown was very loose. He took an impression and shade and sent the case to the lab to have an immediate screw retained provisional made. We saw her for a consult and same day surgery. Tooth #9 was extracted and an immediate implant was placed with an immediate provisional. 4 months later the permanent crown was placed.
